THE SAVED BUDGET OF MEDICAL ORGANIZATIONS IS SENT FOR ADDITIONAL PURCHASE OF MEDICINES

It is known that SK-Pharmacy procures medicines free of charge for the population of the country within the framework of the state volume of free medical care and in the compulsory health insurance system. At the same time, budgetary savings are always brought to the fore as one of the important strategic areas of activity of the Single Distributor.

The results of the savings are very significant. Only in the procurement of 2022, the Single Distributor saved 23 billion tenge, and by the end of the year this amount will increase further, since additional procurements of medicines will be made.

It should be noted that the saved 23 billion tenge will be directed to additional procurements of medicines, which will allow more patients to be covered with free medicines.

In total, over the years of work of SK-Pharmacy LLP, more than 185.9 billion tenge was saved. This result was favored by the centralization of procurements and the consolidation of demand from all regions.

The procurement procedures of the Single Distributor are legally fixed by the Rules for organizing and conducting the procurement of medicines, Decree of the Government of the Republic of Kazakhstan No. 375, which allows the purchase of effective and high-quality medicines at prices not exceeding the marginal prices approved by the authorized body of the Ministry of Health of the Republic of Kazakhstan. In addition, following the results of the procurement, negotiations are envisaged to reduce the delivery price.

Thus, the Ministry of Health of the Republic of Kazakhstan has compiled and approved a list of medicines and medical devices to be purchased from the Single Distributor. In addition to the names of medicines themselves, it also contains marginal procurement prices for each medicine. The budget of medical organizations for drug provision within the framework of the GOBMP and in the compulsory medical insurance system is allocated based on the marginal prices approved by the Ministry.

Savings are achieved due to the centralization of purchases and the consolidation of the volume of demand from all regions. Suppliers go for a significant price reduction when the volume for the whole republic is purchased centrally. And it is this difference between the allocated budget and the amounts actually used that constitutes the savings. The released difference of medical organizations is directed to additional purchases of medicines, which allows to increase the coverage of free medicines for a larger number of patients within the allocated budget funds.

The Single Distributor has effective mechanisms for price reduction. For example, tax-free drugs are purchased without intermediaries directly from the manufacturers themselves, and also through international organizations. The intermediary is excluded from the procurement chain.

At the same time, part of the logistics costs (certification, customs clearance) is covered by the Single Distributor, while in normal cases, when goods are purchased through distributors, all such costs are already included in the cost of purchases through the single distribution system.

The practice of concluding direct agreements and international organizations bring their benefits. This year, savings only under contracts as part of the purchase directly from the manufacturer amounted to about 3 billion tenge.

Savings based on the results of the purchase are also noted when purchasing under long-term contracts with domestic producers, which suggests that the development of the domestic pharmaceutical industry is not only ensuring the national drug safety of the country, but also the possibility of saving budgetary funds.

But the greatest price reduction is achieved through the price reduction auction, which is carried out as part of a two-stage tender for the purchase of medicines. It is this stage, as a result of serious competition between suppliers, that provides more than their fair share of budget savings through procurement procedures.

This is only actual savings, without taking into account those price hikes that could occur due to exchange rate differences, when SK-Pharmacy LLP acts as a kind of buffer, protecting the population from shortages in medicines due to price increases, since early tenders’ procedures allows you to maintain the same procurement prices for medicines, as well as, in the case of direct contracts with foreign manufacturers, international organizations assuming all currency risks.

In order to ensure the transparency of the procurement procedures for medicines and remove all doubts from the public about the activities of the Single Distributor, a permanent video recording of all tender procedures is being conducted. Information on the results of tenders and volumes of procured products is always available on the SK-Pharmacy website and the Medicines and Medical Devices Procurement Web Portal.

It should be noted that the holding of tenders for medicines and medical supplies has been automated since 2018, and the procurement of services for the storage and transportation of medicines and medical supplies has also been automated since 2021.

At the same time, in order to exclude the human factor and corruption risks from the procurement process of medicines, together with Electronic Finance Center JSC (by the Single procurement operator for medicines and medical devices), the MHRK and its subordinate organizations, work is being carried out to automatically review the compliance of applications from potential suppliers, with a minimum participation of the tender commission, including: introduction of the Unified classifier of medicines and medical devices, finalization of the E-licensing system, in terms of issuing certificates (GDP, GMP), integration of the web portal for the procurement of medicine and medical devices with state databases, which will be used to modernize the web portal for the procurement of drugs and MD on the transition to an automated data extraction process.
